For reference sake, I'm going to ctb due to ADHD and severe anxiety that still leaves me unable to function despite medication and therapy.

Here's my draft: I'm sorry. I know you're devastated. You'll never understand my decision, but I've been thinking about this for a long, long, long time. Why survive when I can never thrive? Why live with a brain too broken for anything? I see no other escape from my struggles. Please remember me with happy memories. At least I'm not struggling anymore.

Any suggestions? I want to ease my family's grief as much as possible.

I'm sorry you feel this way, but if you decide to do it: maybe it helps your family to also tell them it's not their fault? (if it's true)

I wish noone had to CTB. I'm sorry for your struggle with mental illness. I suggest adding something like: nothing can be done and it is noone fault. It has been a well-planned decision without pressure from anyone

I feel like what you have already said is well written. For me I see the purpose of notes as being that those left behind have some closure and they are not left with unanswered questions. I plan to say things like there is nothing you could have done, and now I am at peace. I wish you the best.
