Any records of failed attempts with N?

Are there records on this forum of failed attempts with N?
By failed attempts I mean drinking a full 100 ml bottle and.. wake up. (vomiting is excluded)

Can only think of two on here and both were user error.
First one didn't drink the whole thing. He sipped it slowly before passing out and was found by hotel staff in his own vomit. He also mixed it with Xanax.
The other was someone who was found by family members but their N wasn't from D and there was speculation that their N powder wasn't pure.
Both were quite a while ago.

Oh and several where people got it all ready but couldn't drink it. But that's SI I guess rather than a failure.

xLosthopex was a famous example. She was overwhelmed by the taste and the quantity of N that purportedly needed to be consumed. Many replies in her thread were quite unpleasant. She re-purchased and added Stevia and is no longer with us. :(
