A few reasons.

1. In prisons ,they don't use real doctors,they basically pick someone to be the "prison doctor"

2. They don't just use Pentobarbital. They also use something to paralyze and stop the heart of the person. Absolutely no reason whatsoever to do this other than save the viewers from seeing the death seizures of the body. It's definitely excruciating if the sedatives haven't kicked in , and maybe even after.

In short, they're idiots who don't know what they're doing. The prison system in the US is a catastrophe.

That was horrifying to watch. But near the end I think I heard the one in the white coat say that its 'in his tissue'. I think that means they didnt actually inject it correctly? Im not sure who said 'im burning up' but if it was the prisoner I would guess that's what happened and they messed up severely. I probably wasn't focused enough due to the borderline traumatizing experience of realizing he was in pain from the grunting to properly focus on who was saying what and I'm probably not going to re-watch so you can take my comment as a grain of salt.
