This is an interesting article to read. I believe that the man's health conditions did contribute to his near death. In order to be lethal, I would say that a large enough dose of tetrodotoxin must be ingested in order to reliably CTB. Also, keep in mind that it would be uncomfortable (or possibly painful) since your body would be paralyzed, unable to breathe on your own, basically suffocating to death as your muscles paralyze (including your lungs) as the toxin is in your body. There is no cure for this, however, people have been saved when they are given medical aid until the toxin works it's way out of the person's body.

It is definitely painful, you are fully conscious while you can't move and breathe.
