Ive been in 3 sessions with my therapist and im going to have the 4th one. I dont feel like this is going anywhere, because i might need something more drastic. Should I just tell hell that im suicidal and all that? If i tell the truth, im propably gonna end up in a phych ward, and i dont know if this would be for the best or for the worst. But I dont know what else to do at this point.

Okay I know exactly how you feel. The exact same thing is happening to me rn. Tm will be my fourth appointment and yeah Same for me it's not going anywhere and it feels like it's not working.

But to answer your question, I think you should tell your therapist you are suicidal. That way they can help you. And just because you express that you are suicidal -- you don't automatically end up in a pysch ward. From what my therapist told me, when a client expresses these feelings they have to assess them and see how severe they are and if that person has an active plan in place and is set on going through with said plan. Even then, if the person has a plan my therapist told me for example if I did have a plan and was going to go through with it she would try to basically talk me down and get me to calm down and stuff before taking drastic measures like that.

Sorry for the yap but tldr - I personally think you should tell your therapist and a pysch ward is seriously a last last last resort option (like if you're an actual danger to yourself and will immediately harm yourself and nothing else is working). But I have to say this based on what my therapist told me and I am based in the USA

Why do you want to do it? What do you expect that it will happen? Do you think that they are not taking you seriously? It could help, but I'm not sure if it's that useful. After all, in therapy you need to talk about your problems, your traumas, your fears, etc. I feel like, if you think that this is going nowhere, you should simply tell them that. That you feel like therapy is not working and you may need change the way that your sessions are going. They may tell you that it's too early to say (three sessions are not much, after all), but it's something important to mention, I think. If after several sessions more you are still not feeling anything, maybe you should talk to them about changing to another therapist.

But about your question, yes, you can say that you sometimes think about suicide if you want. It's unlikely that they will send you to a psych ward if you don't talk about plans or about being scared of doing it impulsively. After all, the psych wards are not big enough for every suicidal person.

I definitely think you should. But as others have said before, NEVER say you are planning to act on any plan. That is the golden ticket to the psych ward.

My short answer is yes, tell your therapist. But maybe start slowly and maybe don't tell you're actually plan to do it really soon and like there's nothing that could stop you. ;)

I trust my therapist fully. But I built that trust over years. She spent years seeing how my suicidal ideations were growing in me. She was there to hear when I think it started in me for real. I told her how I'd love to just not being and how it would be a solution to cease my pain. Every time I spoke about it I'd be a little calmer. After maybe 100th time I was able to say it without my voice breaking and eyes crying. Sometimes she looked really devastated seeing my pain and the way I elegantly spoke about it and with clear conclusions. And she still didn't call anyone to lock me anywhere. One time she asked me if I plan to do it and I answered "no", even if it sounded like bs and right after that I told her I really don't want her to call anyone. I also told my therapist over time that I wouldn't tell her if I was to finally end me, because of that. It was coming from my trust and respect towards her. I don't know if I would trust my therapist with it this early, but I do feel one should tell and so do you. Generally try to help your therapist help you.

I once told my psychiatrist that I had a suicide plan and she medicated the fuck out of me. No psych ward, but I would not advise doing what I did.

I think telling about feeling like you don't want to exist anymore or that you want to die is generally ok as long as you don't mention a plan or ideas to materialize such desires. However, in my experience, no therapist has been able to really help me about suicidal ideation. I've always felt the conversation turns uncomfortable, and I think that the taboo around suicide is to blame.

Are you scared that you are going to CTB impulsively? Or to do something else, like hurting yourself or another person? Because if so, yes, you should tell them.

Another question would be, do you think you need to go to a psych ward? Or to receive a lot of medication, like @TurboCharcha said? Because we are answering on the assumption of that you simply want therapy, but if you are scared of doing something you don't want, maybe you need something else too.

I don't think anyone can truly tell here what you want since we have very little information.
But in a general sense for recovery I would say being open helps the most.
