Medically assisted suicide

Have you checked out different websites? Dignitas or Pegasos? Usually their regulations are strictly defined though. Not sure about Canada.

Are you terminally ill, do you have a life altering condition that impairs you without suitable cure? Or do you simply just want to die? Do you also have a suitable amount of funds? Assisted suicide is expensive and selective. You have to go through an application process that will require medical records. Dignitas and Pegasos information is largely available to the public. Canada currently has stricter MAiD access so that isn't a generally sought after avenue.

Do any places consider mental illness.. i have physical disabilities that effect my quality of life. I want to stop suffering after 10 years of treatment for mental illness. How expensive?

For mental problems only countries are belgium and the netherlands i believe but only for their citizens and they must be not treatable, for physical conditions there are more countries like switzerland or canada but you must be terminally ill or have a very very serious condition like being a quadriplegic
