It's a possibility, or you could pass out without feeling pain. If there's an intervention, then there can be pain because the antidote(Naloxone) coincidentally counteracts the opioid's effects. The answer might be dependent on how susceptible you are to passing out when you take it.

I've never used heroin but I was overdosed on opioids (in a medical setting, which gives you an idea of how easy it can be) and even if you don't immediately pass out I doubt it'd be painful except for the initial injection.

I was basically forgetting to breathe and almost going unconscious with zero pain/discomfort except for dizziness; the only reason I stayed conscious was that someone reminded me every few minutes to breathe. I also was in no pain at all even though I'd just had major surgery.

Obviously it's not identical to heroin but most opioids are pretty similar in their main effects like that and the experience has made me totally unafraid of an opioid overdose. So in my view with this method (assuming you can get it into a vein) the only outcomes are getting high, being found, or dying with minimal pain/discomfort.
