Due to the fundamental difference in the mechanism and speed of their effect on the body.

A detailed comparison of the processes of poisoning by these substances is provided below.

Comparison of the effects of substances

Criterion Sodium cyanide (NaCN) Sodium nitrite (NaNOâ‚‚)
Speed of action
Instantaneous. Symptoms develop within a few seconds; loss of consciousness lasts from 30 seconds to several minutes.
Slow. The process takes anywhere from a few tens of minutes to several hours.
Mechanism of toxic action
It blocks cellular respiration (the enzyme cytochrome oxidase). Cells instantly lose the ability to utilize oxygen.
It oxidizes hemoglobin to methemoglobin, completely depriving the blood of its ability to transport oxygen to the tissues.
Excruciating symptoms
Brief convulsions, asphyxia, sardonic smile, coma. Subjective perception of pain is rapidly lost due to the deep state of unconsciousness.
Prolonged, progressive suffocation, severe vomiting, unbearable headache, a drop in blood pressure, and convulsions while remaining conscious.

Why does sodium nitrite cause a more agonizing death?

1. Prolonged consciousness: Upon ingesting sodium nitrite, a person remains conscious for an extended period, experiencing all the symptoms of cerebral oxygen deprivation. Cyanide, in contrast, causes a loss of consciousness almost immediately, meaning the victim does not have time to suffer physically for long.

2. Severe physical symptoms: The effects of sodium nitrite include intense gastrointestinal irritation, agonizing nausea, and incessant vomiting. Concurrently, rapid vasodilation leads to a critical drop in blood pressure and a splitting headache.

3. The "drowning in air" effect: The blood of a person poisoned by sodium nitrite turns chocolate-brown due to methemoglobinemia. The individual breathes rapidly and deeply, yet their tissues and brain slowly die from hypoxia, triggering an intense sense of panic and suffocation.

Medical aspects and risks

Both substances are extremely dangerous toxins, and exposure to them requires immediate medical intervention.

Sodium cyanide: This substance blocks the ability of cells to utilize oxygen, leading to rapid systemic organ failure. The primary danger lies in damage to the central nervous system and the cardiovascular system.

Sodium nitrite: In high concentrations, it causes severe methemoglobinemia, a condition in which hemoglobin loses its ability to transport oxygen. This results in profound tissue hypoxia, a critical drop in blood pressure, and brain damage.

Any suspicion of contact with or ingestion of these substances constitutes a medical emergency.

There is also some witnesses here showing that, for sure, it's at least not always peaceful. There is cases of puking, agonal breathing... Not everybody want to leave this way.
But there is others methods

There is no suffocation. Painful feeling of suffocation is caused by excess carbon dioxide in the body – receptors detect this, resulting in intense pain. During the SN method, your lungs continue to exhale carbon dioxide, so your brain doesn't know you're suffocating. You do not feel it

I kindly invite you to read this tread https://sanctioned-suicide.net/threads/first-ever-sn-documentation.29734/

Which is one of the fewest documented SN ctb.

It seems it was far from peaceful. Here are some words according to the witness : throwing up, groaning noises, gasping for air, leg spasms

Nothing very peaceful. Also the death took 20 long minutes, so far from a 30 seconds hanging. In my opinion, a 20 minutes death is agony, not death.

Ofc, it's only a witness post, unlike hangings, there is few videos and documentation about SN so it's hard to be certain, and this case can be the exception while many other went peacefully, but doesn't look peaceful imo.

Ai also said that Sodium Nitrite causes the person to feel like he is drowning on a dry land (gasping for air) which we all know it isn't true.

Puking is unavoidable sadly, as for the agonal breathing you won't be conscious to feel yourself doing it.
