I'm using a belt that's pretty thin and it's almost made me pass out in many practice rounds I did. I was using a kneeling position, too.
I think its relatively less important than the position of the knot behind your head and whether or not you're actually pressing against your carotid arteries. Based on what I've seen here, people's carotids differ in location and there's no universal position.
I saw a guide here that said to put the loop above your Adam's Apple and it did not work for me because it turns out my carotids are just right beside it lol.
