Imagining a scenario where a person shoots himself in the mouth with a 9mm pistol (type of shot to the temple with a hole that allows sewage full of viruses, fungi, bacteria, etc.), and ends up in a tank full of sewage, how difficult or impossible would medical interventions be? What are the chances that doctors would decide not to operate because of complications, such as serious infections (present and certainly future (and irreversible brain damage?

If they found you alive they'd probably make the effort and save you. Your life after the act may be shittier due to the infection though. I'd keep it simple and do the 9mm correctly and avoid the sewage.

Thank you for your reply, but I'd like to point out that my question was more technical and specific in nature. It was about the medical difficulties involved in a scenario of severe contamination (viruses, bacteria, etc.) and brain damage resulting from a non-lethal gunshot.

I understand it's a sensitive topic, but I was looking for a more objective analysis—hypothetically—regarding survival chances, infection risks, and neurological damage.

If anyone has medical knowledge or experience in healthcare, I'd really appreciate a response that focuses more on these aspects.

Antibiotics are an amazing development in the medical field and work quite well, despite all the horror stories you hear about MRSA. If your shot placement is not correct and you wind up with a major infection in your sinuses, gunshot wound and surrounding areas you will most likely wind up unable to care for yourself (or attempt CTB) again.

Like the poster above said, don't count on the sewerage to do anything other than just fuck your life up worse than a misplaced gunshot wound will.
