Question 1:
Does anyone know or have pictures on how to properly secure the metal ring? This includes how to tie the knot and how to thread the rope? The metal ring I have is 5mm thick with an inner diameter of 30mm. The nylon rope is 12mm in specification (though after my stress test with an 80kg load, it stretched to about 10mm).

Question 2:
Finally, when wearing it around the neck, is it necessary to place a soft barrier, like a paper towel or cloth, between the metal ring and the neck?

What is the metal ring for? You should tie a noose. You don't need a metal ring.

Do you want to use the ring to form a noose? In that case, you need to tie one end of the rope securely to the ring, and thread the other end through the ring. This will form a noose.

To tie the noose to the ring, any strong, secure knot works. There are countless options. Here are some simple ones:
If the ring is large enough to fit the rope through it multiple times:
Any of these would work.

Whichever knot you choose, I'd also add a stopper knot at the tail end. Adding one never hurts. It only makes the knot more secure.

It isn't strictly necessary. I wouldn't put a paper towel or soft cloth there, because it could get pulled through the ring and prevent the noose from tightening correctly. If you're afraid that your skin might get pinched by the ring and the rope, you could place a piece of cardboard there instead. I think a harder material is better due to the reasons above.

Looks okay to me. I could only guess what exact knots you're using, but they look nicely tied. :) As long as they hold well, this should work.

Looks okay to me. I could only guess what exact knots you're using, but they look nicely tied. :) As long as they hold well, this should work.
The ring: Arbor knot + Stopper
The anchor: Hitch knot + Stopper
