who's going to put you under? Alcohol, opioids, and sedatives can add to the effect of anesthesia, sometimes resulting in respiratory failure. But the med staff won't let you go that way.

If you expect a surgery and anesthesia - even you don't follow what they tell you to do (like no water/food/drugs and all that) If there's sth going wrong they will interfere and rescue you within seconds long before you die. Modern anesthesia is pretty much "secure".

No. They do a blood and urine test before surgery. They would know if you took something that you shouldn't have. They won't do the surgery if everything doesn't come back alright. You have a lot of people watching you carefully during the surgery. They would know if something was wrong and would intervene. They aren't going to risk losing their license and getting sued if you die.

Everyone who is put under anesthesia has a "team" in the operating room. No matter what you take they will have on standby. If you are that out of your baseline it will raise red flags to your surgeon and the procedure maybe canceled. You would have to fool the surgeon 1st and by time the anesthesiologist comes they are putting you out. The one you definitely can't fool is the anesthesiologist because they will ask you a few questions and if you are barely conscious and can't answer them, they won't medicate you. But if for some reason you can fool them, they will have monitors watching your vitals closely. They will have emergency medication on standby and can intubate you if necessary. I highly don't advise this method. It's like going inside a police station and stealing a fax machine and trying to walk out..it won't happen.

I hesitate to use this word, but OP's idea really is stupid for just this reason.

You're surrounded by up to a dozen medical professionals, one of which [the anesthesiologist] is watching your vital signs the entire time.

You could drink hemlock before surgery. They couldn't detect it and there is no antidote. You would die under anesthesia and it would take them a while to figure out what your seizures and tachycardia were coming from. In an attempt to stabilize the heart they could anticipate death. But you should do it a few minutes before anesthesia and if it's postponed you're screwed.
