I do know of a young woman who died while diving. So that is one way - not sure it's kind to the instructor, but it's an option.

Are you in it for the gamble/kicks or is it life isurance related ? If the latter , it has to be believable so the more in line with things you would normally do the better. Makes it harder to deny payout.

Depends , some places require 10 tandem jumps some 20 etc.
