It's quite easy to black out but I always end waking back up.
The only reason for this I can think of would be that after passing out I sort of unconsciously convulse which leads to subtle changes in position, thus pressure on my arteries end up loosening or whatever..
But I don't know what happened after I blacked out so it's all guesses..

From my experience, the success rate seems so low and I wonder how people actually get this to work all the way

I couldn't really pinpoint one from your picture so I'll use the picture I found.
I don't think this one's exactly identical either but iirc I've tried positions similar to these two
Judging by this picture, I think you are failing because of the lack of pressure on your neck.
Either the placement of noose is not in between hyoid bone and mandible or the rope is too thick in comparison with your weight.
