Why do people ask that so often even though most of them don't actually want you to respond honestly if you're feeling bad? I don't understand the purpose of it

Just simple common courtesy, I think. You ask people how they are because that's just what you do in conversation, the same way you ask someone "hey, what are you up to?". But people don't consider the true ramifications of asking such a question, which of course leads to endless scenarios where somebody has to say they are doing alright when they are not. Happens to me so often... You'd think by now my response would be automatic but each and every time I do imagine what would happen if I just told the truth.

Most times I just reply "as usual" since it's not technically a lie and they don't know how bad the usual is

I got sick of that and often I will say I don't feel fine and then there's an awkward silence.
