Help Can't find "D"'s contact info in the PPH. Where is it?

I've gone over the latest PPH and can't find ways to contact D. (As a side note and possible advance apology I suffer from advanced macular degeneration so it's possible I'm staring right at it and not seeing it. ) But I've looked. Thanks.

I can't find it either! I bought the print book, nothing there either — I thought something must be wrong with me!!
