Please call a Community Health Center in your area. You will be assigned a Case Worker to assist to receive SS.

You can can SS, directly to open your Case. Give them the names, addresses of Doctors.
No need for you to incur expenses for medical records. SS will get them.

Coupling, the Mental Health facility Case Manager with the application will get you qualified, faster. They will go with you to appointments set by SS.


There is no need for an attorney. Patience is required, it's not instant. It's a journey.
